well, for starters,

welcome, you chose the right place to find information on just about EVERYTHING concerning a luv....

that being said,

the search function in the upper right hand side is a great tool. look up phrases like "5 speed swap", "Different Trans", or something of the like

Also, check out the drivetrain upgrades forum as there are lots of threads that deal with that specific issue.

but let me tell you right off the bat, your only options for running gear is Isuzu Troopers, and some later Pups.

also check out the 4x4 forum for some info too.

again, Welcome!

You have Isuzu corporate differentials, 8-bolt front, 10-bolt rear, 4.10 ratio. The only other ratio available for the front is 4.56, available in '84 through '87 gasoline powered Troopers. Both Trooper differentials will retrofit. New gears are not an option, even if you found them they would cost more than your truck is worth and would have to be mounted into your stock housings- even more expensive. Swapping in Trooper rears would give more power at the expense of mileage. The rear third member swap is simple, the front requires dropping the whole axle assembly.
23 mpg at 50 mph is excellent- you have to remember when these vehicles were built the oil embargo was in effect, the national speed limit was 55mph and all vehicles were designed to give adequate power and maximum efficiency at that speed, they were never intended to be driven at current interstate speeds, much less be fuel efficient at those speeds.
The 4x4 5-speed behind '86 & '87 2.3L pickups & Troopers could be swapped in with a few mods, but your smaller engine may not have enough power to use 5th gear at cruising speeds.
There are some engine adds available like cam, headers, 1.8 head on a 1.9 block & bigger carb, you might get 20% more power, and the later 2.3 or 2.6 engines can be installed and then modified further, but that's about it without a complete drivetrain replacement.

Your best bet with a 4x4 is to go with a taller tire (skinnier preferred because you don't want to kill the engine with too much rotational mass) and a 5-speed... however, if you live on any kind of an incline 5th gear is going to be useless.
