Lots, and it depends on the year, and even the specific 'package' sometimes. Some of the differences can add up to an unexpected surprise when you're trying to replace parts, which I learned the hard way. It's not too bad, really. They've just got some extra

Short list of differences on but not limited to the Mikado option:
auto choke
hydraulic clutch instead of manual cable
multiple electrical options
a few 'deluxe' interior upholstery options
steering wheel
what am I forgetting?

Your best reference in this regard is the Master Parts List. On several pages, you will find "Mikado Specific" parts or something of that nature. Unfortunately, I don't think these pages are specifically identified in the table of contents, but the 'interior' section of the book has 3 solid pages of alternative parts. Somebody correct me if I'm wrong.

Ok so it's mostly an appearance package?

Yes mostly creature comforts. Cloth seat and door panels, dome light that is door switched, chrome strip on top of door panels to name a few.

Never heard of it having to do anything with the clutch and choke. All interior options.

Yep. My 76 non-mikado has a hydraulic clutch and my old 80 mikado had a cable clutch. Both of them had automatic choke.

Mikados had a/c, fuzzy door panels, carpet, plush seats, a smaller steering wheel with a 2 note horn... Stuff like that.

A/C was a separate option and Mikado had nothing to do with the clutch. The hydraulic clutch was used up to 77, then they switched to a cable clutch on all trucks.

From what I recall these were the Mikado upgrades:

Cloth and/or upgraded seat with color accents
Door panels have a matched (to the seat) fabric panel in the middle and a carpeted extension to the bottom of the door
Carpeted floor
Colored kick panel covers (I think these may have only come in 78 and later Mikados, my 79s had them but 77s didn't)
Larger dome light with door switches
Faux leather gear shifter knobs matching texture of the steering wheel
Three spoke steering wheel with faux leather texture (The best Mikado option IMHO)
Mikado emblems on the back corners of the bed and on the glovebox door
AM/FM radio may have been part of the Mikado option package
Two note horn
Almost everything was interior changes, no mechanical changes at all.

On the 81-82 models I think the Mikado was painted/Decal on the bed and might have included a stripe package too. I know they had a "Sport" package for those years that was similar that had paint/stripes too.

editted: I forgot the two note horn that drummer mentioned
