It is the Air Injection Reactor (AIR) system. The belt driven bit is a pump that puts fresh air into the exhaust to help burn any last traces of unburnt hydrocarbon emissions as they go out the pipe. It does not give any horsepower (actually uses a little bit to run) but it does cut the emissions you are putting out. If you have to pass smog testing, it is usually required to be in place. Do you need it? That depends on if you can get the truck registered and pass smog without it.

They also don't seem to take well to sitting idle from what I've seen. Might just be the wet PNW air, but I have not had good "shelf life" from them. I replaced one twice with a good working used pump that had been sitting around for a few years and both replacements didn't hold up long before the bearings went out. This is one item that if you need one you are going to do better with a new/rebuilt or one pulled from use very recently.
